We ask you to complete the new patient form packet prior to your first visit.
If not completed, please arrive 20 minutes prior to your appointment. This ensures that we have all the relevant details .
We will need your insurance information.
Navigating insurance requirements can be complex, and we will do our best to assist you. Understanding your insurance plan is crucial, and some policies may necessitate a referral from your Primary Care Physician (PCP) or prior approval from your insurance provider for various procedures. While we strive to provide support, please contact your insurance company to familiarize yourself with its policy and specific requirements for care approval.

Referral information.
Fax referral to: (484) 328-6491. We will do our best to help you with your referral prior to appointment. Patient may be responsible for any charges for services prior to referral is received or has expired.
Please include the following:
- Patient name, demographics, contact information and insurance provider.
- Reason for referral.
- All relevant office notes, laboratory data, and imaging reports as needed.
What to bring for your appointment?
Please bring:
1. Your Photo ID
2. Your Insurance Card
3. A form of payment (copay/deductible/co-insurance)
4. Your referral
5. List of your current medications, previous records from allergy physician or other providers including any lab or x-ray results.
What to expect?
Meet with our physician, who will throughly review your medical history and any records, discuss your current symptoms or concerns, and perform comprehensive examination.
Based on consultation, diagnostic testing and procedure will be recommended. This can include skin testing, blood tests, advanced imaging, endoscopic evaluation, and lung function testing.
A treatment plan will be recommended based on your diagnostic results along with your symptoms.
What is your cancellation policy?
To cancel an appointment, please call the office 24 hours prior to your appointment.
If you miss your appointment or cancel with inadequate notice, you may be charged a cancellation fee.
We understand that circumstances may arise beyond your control and appreciate you communicating with our office as soon as possible.

Insurance and Billing
As part of our commitment to patient care, we manage primary and secondary insurance claims. Don’t forget to bring copies of your insurance card(s) front and back to your appointment. Additionally, please provide the subscriber’s birthdate, employer, and social security number if they are included in the insurance identification number.
Insurance Information
Payment of co-pays or deductible is required at the time of your appointment.
PLEASE BE ADVISED: While we strive to assist our patients in navigating their insurance coverage, it’s important to recognize that both you and we are bound by the policies and limitations set forth by insurance plan. While we aim to aid in understanding your insurance options, the ultimate responsibility lies with you. Many insurance plans do not necessitate a referral, and we recommend that patients verify this with their insurance provider.
Regrettably, the submission of insurance claims does not guarantee payment by the insurance company. The obligation for payment of services rendered falls directly upon you, irrespective of your health insurance coverage. Any outstanding bills or unpaid amounts by your insurance will be your responsibility.
Should you have any inquiries regarding your insurance coverage, we encourage you to reach out to your insurance provider before your appointment.
*** It’s essential to note that most insurance policies permit medical claims to be submitted for reimbursement within 365 days from the date of service. Requests for corrections or appeals to processed claims can be made within 24 months from the original processing date. Your insurance company is obligated to provide an Explanation of Benefits detailing the processing of all service dates, which should align with any outstanding balance statements from our office. If you haven’t received an Explanation of Benefits from your insurance company, please contact them and request one. Please inform us if you have any further questions or concerns.

Only when referral is on record at Keystone Allergy and Asthma Center, an appoinment can be scheduled.
Being aware of the restrictions of a referral and contacting your Primary Care Physician for a new referral when necessary are your responsibilities. Not adhering to this may lead to personal liability for all expenses incurred.
If your secondary insurance requires a referral for specialist visits, please ensure you obtain one to facilitate claims processing and payment by your secondary insurance.

Schedule your telemedicine visit. At this time, telemedicine visit is only provided to existing patients residing in Pennsylvania at the time of the visit.

We strongly advise all visitors to wear surgical masks during their visit.
> Patients who have tested positive for COVID-19 within the last 10 days or exhibit symptoms such as cough, fever, sore throat, or nasal congestion – Please call our office before your visit.
> Visitors who have tested positive for COVID-19 within the last 10 days or are experiencing symptoms – Please call our office before accompanying patient to office.
Face masks are required for all of our immunocompromised patients.
